ChanceLight® Autism Services: Report a Grievance

Hi ChanceLight family, if you are having any concerns or complaints about services or treatment, please choose one of the below options:
- First, try and contact your Program Supervisor or Clinic Director directly and they will return your call within one business day. To access Clinic contact information, please click here: https://autismservices.chancelight.com/locations/
- If you prefer to submit a report online: Please click http://www.hotline-services.com. There will be a space called, “Organization Name,” and you will simply type “ChanceLight”, click the “Submit New Report” button and follow the prompts to complete. Once you have submitted, please make sure you save your 9 digit PIN number for tracking purposes and someone will be in contact soon. (Please note: You are welcome to remain anonymous if you like)
- You may also call our hotline at 855-252-7606 and either remain anonymous or request a return call.
